    All Brecon Canoe Club members MUST (after a short period of grace) also become Members of Canoe Wales or another UK National Governing Body of paddlesport (British Canoeing / Scottish Canoe Association / Canoe Association Northern Ireland). The minimum level is Club Associate Member, which provides third party insurance and legal expenses cover for paddling activities with the Club and costs just £10 (adults) or £5 (under 18).     I consent that photographs or video taken by authorised personnel of myself/ my child for the purpose of coaching aids, record of events, publicity on leaflets/websites/magazines/ published articles, future grant applications and Social media sites including Twitter and Facebook
    I want to Gift Aid any donations I make in the future or have made in the past 4 years to Brecon Canoe Club (including the Club’s annual membership subscription, which simply gives the basic rights of membership and does no more than cover the basic administration costs of the charity and is therefore distinct from any fees paid for participation in activities provided by the club). I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all my donations in that tax year it is my responsibility to pay any difference.
    I confirm to the best of my knowledge that I / my child does not suffer from any medical condition that might affect my / their participation in paddlesport other than those listed on this form. I have completed this form accurately and included all details that might be needed by the Club to work effectively with me / my child. I acknowledge that paddlesports (including canoeing and kayaking) are assumed-risk, water-based contact sports and may carry attendant risks of personal injury. I agree to abide by the rules of the club and to adhere to any instructions or advice given by activity leaders including the use of equipment. I agree to the Club holding and using my / my childs personal information in the way and for the purposes described on the registration form. I consent to myself / my child receiving appropriate first aid or, in a medical emergency, consent to medical treatment which, in the opinion of a qualified medical practitioner, may be necessary.
